Introduction

Stroke Recovery — The First 6-12 Months

Stroke is a leading cause of death and disability in Malaysia. A patient just home from hospital usually needs 6-12 months of active recovery at home — with the right equipment, therapy, and a consistent routine. 🏥

This guide condenses what doctors and therapists recommend for families newly receiving a stroke patient at home.

Essential Equipment

1. Electric Hospital Bed

Stroke often weakens half the body. The 3-function electric bed lets patients sit up to eat, lie back to sleep, and be raised for cleaning without straining the caregiver.

2. The Right Mattress

For fully bedridden stroke patients, use the anti-decubitus air mattress. For patients who can still turn, hospital-grade foam is enough.

3. Wheelchair

For transfers between bed, toilet, living room, and doctor visits. We recommend a foldable model for easy car loading.

4. Cane or Walker

When the patient relearns to stand, a wheeled walker is safer than a single cane.

Recommended Daily Routine

Morning (6:00-9:00)

Slow wake-up with the bed raised. Sponge bath in bed with warm water. Light breakfast with the head propped at 60°.

Midday (12:00-14:00)

Lunch and post-meal medication. Position rotation (right/back/left) to prevent sores. 1 hour of rest.

Afternoon (16:00-18:00)

Light hand/leg exercises (see therapy section). Prep for dinner. Diaper change if needed.

Night (20:00-22:00)

Another sponge bath, apply moisturiser. Sleep position — usually head raised 15-30° to reduce reflux.

Recovery Therapy at Home

Passive Exercises

For patients who can't move yet, the caregiver gently moves the limbs daily (shoulder, elbow, wrist, knee, ankle). 10 minutes per session.

Assisted Active Exercises

When the patient starts moving a little, ask them to lift limbs with minimal help. Don't push — 5 small reps beat 20 forced ones.

Speech Practice

Many stroke patients have speech issues (aphasia). Name simple objects, read newspapers aloud, or sing old songs together.

Avoiding Complications

Bed Sores

Reposition every 2 hours. See our bed-sore prevention guide.

Urinary Tract Infection

Common in catheter patients. Keep the catheter bag below bladder level and replace every 2-4 weeks.

Pneumonia

High risk in patients lying flat. Always raise the head 30° after meals and have the patient cough deliberately 5 times daily to clear lungs.

FAQ

How long is stroke recovery?

The first 3-6 months are most critical. Many patients keep recovering up to 12-18 months with consistent therapy.

Do I need a nurse to come home?

Not mandatory if family can be trained. For patients with surgical wounds or catheters, a nurse 1-2 times a week helps a lot.

When can walking practice start?

Up to the doctor — usually after the first 2-4 weeks. Don't push; use a walker early on.

The patient cries for no reason — why?

Many stroke patients have post-stroke depression. It's treatable with medication and family support — discuss with the doctor.

Do all stroke patients need oxygen?

No. Only when the doctor prescribes it — usually for patients with lung or cardiac complications. See our oxygen guide.
